Boar’s Head Deli Meat Plant Repeatedly Violated Federal Health Violations, Led to Listeria Outbreak

The Boar’s Head deli meat plant in Virginia that has been tied to the deadly listeria food poisoning outbreak had repeatedly been logged by government inspectors for “noncompliance” with federal rules in the past year.

Federal regulation violations they were logged for included instances of mold, liquid dripping from ceilings, insects, and meat and fat residue found on walls, equipment, and floors. Between August 2023 and August 2024 inspectors found the following:

  • Heavy discolored meat buildup
  • Meat overspray on walls
  • Large pieces of meat on the floor
  • Flies going in and out of pickle vats
  • Black patches of mold on a ceiling within the facility
  • Blood pooled on the floor
  • A rancid smell in the cooler

Documents detail that plant staff were repeatedly notified that they failed to meet requirements. Virginia officials inspected the plant through a partnership with the US Food Safety and Inspection Service, and when problems were found officials said Boar’s Head took “corrective actions in keeping with FSIS regulations.” However, federal reports show that no enforcement actions were taken against Boar’s Head between January and March. 

The US Agriculture Department officials have suspended inspections at the plant and it will remain closed “until the establishment is able to demonstrate it can produce safe product.”

A Rutgers food science and safety expert who reviewed the inspection documents said that the reports of condensation found throughout the plant are concerning as that’s a known risk factor for creating conditions for listeria to thrive. 

So far, the deaths of at least nine people have been linked to the Virginia plant, and the hospitalization of about 50 others across 18 different states. It was confirmed by the New Mexico Department of Health that the New Mexico victim was an elderly, Bernalillo County woman. All 59 victims became sick with listeria after consuming Boar’s Head Provisions Co. Inc. deli meats. 

After tests confirmed the presence of listeria bacteria in Boar’s Head deli meat products was in fact what was making people sick, the company recalled over 7 million pounds of their products. 

According to the CDC, each year in the US listeria infections cause around 1,600 illnesses, and lead to the death of about 260 people. The bacteria can thrive on walls, drains, floors, in the cracks and crevices of difficult-to-reach areas of food processing equipment, and more. It can also easily be spread by pests like flies and the germ is able to survive in biofilms, which are thin, slimy collections of bacteria that can be difficult to get rid of.

Sunday morning on September 1st, 2024, 81-year-old Augustine Youhanna Sana was tragically struck and killed by a hit-and-run driver while walking home from church in El Cajon, California. 

The accident occurred at the intersection of Jamacha Road and Brabham Street, just blocks from his home. 

Witnesses reported that Sana had just entered the crosswalk when a speeding gold Toyota sedan made a sharp turn, hitting him. The driver stopped briefly, blamed the victim for the collision, and then fled the scene. 

Sana was rushed to the hospital where he later died from a brain bleed. 

Authorities are seeking information about the driver, described as a bald man, around 5’5”, and driving a mid-2000s Toyota sedan with faded gold paint.

Defective Takata Airbag Inflators May Spew Shrapnel, 28 Dead in US and Dozens More Injured as a Result

Recalled Takata airbag inflators have now been linked to 28 deaths and more than 400 injuries in the United States. US regulators linked the most recent death to a driver who was killed in 2018 in Alabama. 

The driver was in a 2004 Honda Civic in Etowah County, Alabama. The collision occurred at high speeds, which made it more complicated to definitively determine the cause of death. 

The National Highway Traffic Safety Administration (NHTSA) said it is extremely important for owners of vehicles to replace their recalled airbag inflators as soon as they can. 

Takata used ammonium nitrate in their airbag designs to create a small explosion that inflates the airbag in the event of a crash. However, these airbags have been recalled due to the chemical deteriorating over time in high heat and humidity conditions. This can cause the airbag to explode with too much force, which can blow apart a metal canister and cause shrapnel to fly into the passenger compartment. 

Worldwide, there have been at least 36 deaths due to the defective Takata inflators. The company says that it has replaced or accounted for nearly 95% of the recalled inflators. The potential for dangerous malfunction in the Takata inflators led to the largest auto recall in US history, with a minimum of 67 million inflators being recalled in the US. Worldwide, about 100 million inflators were recalled.

Construction Worker, Joel Olea Gomez, Dies in Construction Site Trench Collapse in Scripps Ranch

A 27-year-old construction worker, identified as Joel Olea Gomez, died in a tragic accident at a project site in Scripps Ranch in the early morning of August 28th, 2024. 

Gomez was from San Marcos, CA, and was working at a construction site across the street from Scripps Ranch High School Wednesday. At around 4 am, a roughly 20-foot-deep trench collapsed onto him, says the San Diego County Medical Examiner’s Office. 

The trench collapse caused him to become trapped under dirt and a large concrete pipe. According to San Diego Fire-Rescue Department Deputy Chief Dan Eddy, this pipe was part of a water system project crews were installing at Hoyt Park Drive and Scripps Ranch Boulevard. 

Gomez unfortunately died at the scene. Emergency personnel had to shore up the trench to prevent further collapse before they could work to remove Gomez. This process took more than four hours. 

Emergency crews needed to remove dirt from the trench using a vactor truck, which is commonly used to clear storm drains. They then worked with engineers to attach a life-pulley system to a ladder truck to safely lower a fire department technical rescue team member down into the trench to recover Gomez’s body. 

A second worker was also in the pit at the time the trench collapsed but was able to avoid getting trapped. Eddy said this second worker was able to get out of the trench without suffering significant injuries. 

Cal/OSHA has been called to the scene and opened an investigation. Until their investigation is complete, construction in the area of the accident will be halted. It is unclear if work on the entire project will stop or just at this section.

On Friday, August 30, 2024, a collision occurred between a semi-truck, a Chrysler, and a third vehicle. The collision led to the death of one woman, who has been identified as 31-year-old Brittany Robbins. 

According to California Highway Patrol investigators, a man driving a Chrysler heading west on Avenue 12, stopped to make a left turn onto Riverstone Boulevard. The semi-truck behind him failed to stop and rear-ended the Chrysler. 

The impact caused the semi to enter into the opposite lane, where it collided head-on with Robbins’ vehicle, causing her death. 

The man in the Chrysler was injured in the rear-end collision. The semi-truck driver was not injured. 

Authorities have stated that neither alcohol nor drugs played a part in the crash.

Saturday morning, a commercial bus traveling from Atlanta, GA to Dallas, TX overturned on I-20, according to Mississippi Highway Patrol. The crash caused the death of seven people and dozens more were injured. 

The bus was traveling westward near Bovina in Warren County, Mississippi when it left the highway and flipped over. Autobuses Regiomontanos operated the bus, and there were no other vehicles involved in the crash. 

The National Transportation Safety Board said on social media platform X that tire failure caused the crash. 

According to the transit company, Autobuses Regiomontanos, they have 20 years of experience providing cross-border trips, and the website promotes the business as a “modern fleet of buses that receive daily maintenance”.

According to a news release, six of the bus passengers were pronounced dead at the scene of the crash, and another died later at a hospital. 

A mother has identified her 8-year-old boy, Kain Gutierrez, and 16-year-old girl, Perla Gutierrez, as two of the deceased. The other five victims who died have been identified as:

  • Miguel Arriaga, 61
  • Victor A. Garcia, 32
  • Moises A. Garcia, 31
  • Elia Guzman, 63
  • Angelica Palomino, 16

Thirty-seven other passengers were taken to hospitals in Vicksburg and Jackson to be treated for injuries they sustained in the collision.  

Authorities will continue to investigate the accident.
